ATHLETE SPORT REGISTRATION

Special Olympics Forsyth County partners with the Forsyth County Therapeutic Program to help us manage the on-going Administration needs of our program.  We are grateful for this Partnership with the Forsyth County Parks and Rec Mgmt Team.

Athlete Medical must be current before the Summer, Fall or Winter Games Kick-off meeting or your Athlete will not be able to participate. Please be proactive!

NEW – Annual Medical Form and Covid Waiver (form below) must be on file for each Athlete as directed by SOGA. NO Doctor appointment is needed. Parent/Guardian will sign the form indicating Athlete is able to compete. The ANNUAL Form must be filled out each year – if you have an active, (3) year medical on file your Athlete is current. When you need to renew, you will use the NEW Annual Form.

If your Athlete’s name is not listed – your Athlete’s Medical is Expired.

REGISTER Your Athlete:

1. Proceed to register your Athlete below by adding contact information. You will need their medical expiration date and 1^st and 2^nd sport choice.  
2. If your Athlete’s medical is expired enter the current date.
   1. Download and Complete the Annual Participation form (formerly titled Athlete Medical Form) and send to Tina Hillmer at SOForsythforms@gmail.com
3. Medical form questions contact: TAHillmer@Forsythco.com.

A current medical must be on file for your Athlete to participate in any Special Olympics Forsyth County activity.

• We will send the medical to SOGA.  Parents, please keep a copy.
• Completed medicals are active for 3 years.

UPDATE: The Cheer Squad is full for Summer Games! Registration for Cheer is closed – so please choose another sport for the 2026 Summer Games!
