Returning Athletes registering for State Competitions:

If you are already registered with Special Olympics Forsyth and would like to register for competitions please do the following:

• Review the list of sports and identify the sport in which you’d like to participate – click here for the list of sports and description
• dentify the registration deadlines by clicking here.
• Contact Michelle Padgett at mpadgett@forsyth.k12.ga.us or 770-887-2461 ext. 202359 with the sport you would like to partipate in.

Steps to Register a New Athlete:

1)      Complete the Athlete Participation Form (click here for a copy), be sure to include the following:

            Insurance Information

            Emergency Contact

            Signature for media release and participation

            Doctor’s Signature giving medical clearance 

 

2)     Turn in the Athlete Participation Form to your child’s teacher (if the athlete is a Forsyth County Schools student) or mail the forms to:
